Employment Transition Generalist

Site: Rockledge, FL

MISSION:

The mission of BAC is to provide persons with disabilities innovative services and opportunities to achieve personal success.

JOB SUMMARY:

Provides guidance and various community linkages to participants and employees participating in BAC programs to support them and facilitate their successful handling of barriers to successful outcomes. Aids and instruction in pre-employment and soft skills training for BAC participants. Participates in community outings. Provides job coaching at various job sites and live and virtual classroom instruction. Provides transportation for BAC participants to and from their home and work or training in agency vehicle.

EXAMPLES OF ESSENTIAL FUNCTIONS:

The list of essential functions below is intended to be representative of the tasks performed within this job. Other duties may be assigned based on business needs.

  • Works directly with participants to teach life skills and to make sure that both individual and agency goals are met.
  • Performs meaningful activities training with the participants.
  • Participates in program outings.
  • Provides safe and timely transportation of BAC participants to their scheduled destinations; remains flexible regarding the daily schedule due to changes in participant needs or traffic issues.
  • Maintains a clean and safe transportation vehicle and work area (example: vehicle, floor, machinery, equipment).
  • Helps provide guidance and job coaching to participants working on designated activities in multiple work environments.
  • Provides departmental assistance to include substitute teaching for classes, both in person and virtual.
  • Ensures active coordination and professionalism in all contacts with consumers, authorized representatives, funding and community sources, and staff to maximize services and resolve issues.
  • Helps participants needing assistance with mobility including those in wheelchairs, on walkers, etc[SH1] .

SUPERVISORY RESPONSIBILITY:

  • None

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree in social work, behavioral science, or related field
  • Two or more years of experience working in social services
  • An equivalent combination of education, certification, training, and/or experience may suffice for the above requirements.
  • Must satisfy Florida Department of Children and Families (DCF) Level 2 background screening requirements.
  • Must have first aid, cardiopulmonary resuscitation (CPR), and automated external defibrillator (AED) certifications.
  • Valid Driver’s license. Maintain a clean driving record and appropriate insurance.
  • May also be required to have or obtain additional formal industry certification(s) based on the area of assignment.
  • May be required to transport clients utilizing their own vehicle and or agency vehicle.

PREFERRED QUALIFICATIONS:

  • Experience working with individuals with disabilities.

In addition to meeting the minimum qualifications listed above, an individual must be able to perform each of the established essential functions in order to perform this job successfully.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Ability to handle confidential information with tact and discretion.
  • Ability to manage multiple priorities to ensure that deadlines are met.
  • Ability to establish and maintain effective and collaborative working relationships with those contacted in the course of work.
  • Ability to regularly attend work and arrive punctually for designated work schedule.
  • Knowledge of the operation, maintenance, capabilities, limitations and safety aspects of transport vehicles.
  • Knowledge of clerical procedures and systems such as managing files and records and other office procedures and terminology.
  • Ability to operate a computer using Microsoft Office products (Word, Outlook, and Excel) and applicable department/organization specific software.
  • Ability to consistently exercise discretion and independent judgment in the performance of the job.
  • Ability to organize work for timely completion.
  • Ability to work independently.
  • Ability to maintain confidentiality of personal information.
  • Ability to communicate effectively orally and in writing.
  • Ability to handle physical aspects of job, including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods of time.

TRAVEL

Travel is primarily local during the business day.

WORK AUTHORIZATION/SECURITY CLEARANCE (IF APPLICABLE)

Must be authorized to access and work on government installations.

PHYSICAL REQUIREMENTS:

Depending on the functional area of assignment, tasks involve the routine performance of moderately physically demanding work, usually involving lifting, carrying, pushing and/or pulling of moderately heavy objects and materials (up to 40 pounds). Tasks that require moving objects of significant weight require the assistance of another person and/or use of proper techniques and moving equipment. Tasks may involve some climbing, stooping, kneeling, crouching, or crawling. These requirements can be done with or without reasonable accommodation.

ENVIRONMENTAL REQUIREMENTS:

Tasks are regularly performed inside without exposure to adverse environmental conditions (example: dirt, cold, rain, fumes). Staff may be required to work outside without air conditioning, depending on job site locations.

SENSORY REQUIREMENTS:

Some tasks require manual dexterity, in addition to visual and hearing acuity.
